Photography / Video Permissions Form The Dorothy Parkes Centre would like to use photographs/videos taken during groups to feature in some or all of the following: • On The Dorothy Parkes Centre Website • On The Dorothy Parkes Centre’s social media platforms including Facebook Twitter and Instagram • In brochures, leaflets, newsletters, and other hard copy marketing materials • Posters inside and outside of The Dorothy Parkes Centre • Roller banner visuals featured inside and outside of The Dorothy Parkes Centre Please note: If you sign the form, we intend to be able to use your images for up to 3 years unless you email [email protected] requesting that we no longer use your image. If we do not have a signed form from you, out of respect of your privacy and to adhere to GDPR we will not include you in any photographs or videos. Please note that by signing completing this form, it is not a guarantee that you will be featured in photographs or videos. I am willing to help with The Dorothy Parkes Centre promotions through the following method(s). I understand that I am under no obligation to take part in any promotion and that I can email [email protected] to state that I no longer want to be part of The Dorothy Parkes Centre’s marketing activities at any time, and I will then be excluded from existing (where reasonably possible) and future online and hardcopy promotional items:
