Job Title: Community Navigator

Salary: £10.00-£11.50 an hour dependant upon experience

Contract: Fixed term until 31st March 2023

Hours: 25 hours a week

 

The Smethwick Community Offer provides preventative support for Smethwick residents aged 18+. Navigators will respond and interact directly with people to support their individual needs in a flexible and responsive manner by providing an initial strength based conversational assessment, information, advice, guidance, social prescribing and signposting as well as low level practical support for up to 12 weeks.

 

Duties will include but are not limited to:

• To take referrals from the “single point of access”.
• Develop a knowledge of local services to enable the individual to access a range of services to meet their needs.
• Liaise with a range of multi-disciplinary professionals who are involved in the individuals care and wider if need be.
• Keep accurate and up-to-date records of contact with individuals.
• Identify when there is a need for urgent action or for a step-up in care and alert the relevant professional(s).

 

Essential requirements of the role :

• Experience working within the community or social work.
• A full, clean driving licence with access to your own vehicle
• If successful, must consent to an enhanced DBS check (fees covered by the organisation)
